History

Transition Home Healthcare was founded in 2010 and was formerly part of Legend Healthcare. In 2016, Transition divested other assets to focus on home healthcare. Transition has found success in the saturated Houston home healthcare market due to our patient-centered approach. Before any decision is made, even from the corporate level, we ask “how will this impact our patients?”. We are dedicated to acting with integrity and doing the right thing every time. In the Houston area where therapists and nurses have many choices as to where they work, Transition’s track record and values allow you to know that our staff and patients are the heart of what we do.
